Kelly L. Forchione, 85, of Woodstock, died Saturday, February 14, 2015 in Crystal Lake.
Survivors include her children, Patrick Cook, Kevin (Gabriella) Cook, Karla (Ed) Urias, and Karmen Aceves; daughter-in-law, Penny Cook; and many, dearly-loved grandchildren and great-grandchildren.
The graveside service will be at 1 p.m., Saturday, February 21, at Oakland Cemetery, Woodstock.
